FORM TES-SC - TRAINING AND EXPERIENCE SUMMARY
This form needs to be completed for your employment history starting with the most current and working backwards for either 3 years (TER) or 10 years (TEO).
This form is a summary of what you completed in our form 5 and/or form 6.
You must state the period of time with each company and what role you performed at that company.
The next column is responsibility A -E. Select one of the following
A: Being Exposed - Undergoes induction, observes processes, work of competent practitioners
B: Assisting - Performs specific processes under close supervision
C: Participating - Performs specific processes as directed with limited supervision
D: Contributing - Performs specific work with detailed approval of work outputs
E: Performing - Works in team without supervision, recommends work outputs, responsible but not accountable
The next column indicates which form you completed. Was it the TER form or the TEO form?
Row ‘x’ on the form is to show any time you were not working in your field of registration (i.e. Fire Detection)
