FORM SC1.1 GENERAL INFORMATION
This form is to be completed with all your personal information.
Section 1 - is for your name and address and personal information.
Section 2 - Fill in the highest qualification level you have achieved and where this was achieved.
NOTE: This page must be signed by a Commissioner of Oaths
Section 3 – Here you state if you have served an apprenticeship and with who
Section 4 – Here you fill in any previous registration or applications made with ECSA
Section 5 – Show any associations you belong to (e.g. FDIA, SAQCC FIRE)
Section 6 – Indicate the registration fee you paid and how it was paid
Section 7 – Here you need to submit three references who will vouch for you, one of these should be ECSA registered already.

Section 8 – Complete a declaration that you will abide by the rules laid down by ECSA
